Please select the grade level your child will be entering in the Fall. Also, please ensure you are signing your child up for the correct section of camp.

Please include first/last name of child's parent or guardian. This person will be the primary emergency contact for your child while at camp.

Include the phone number you wish to be contacted at during the day concerning your child while they are at camp. This will be the first number called in case of urgent matters.

Please indicate any additional adults that may be called in an emergency should the primary guardians not be available. ONLY Adults listed here will be permitted to sign a student out of camp. Adults must show ID when collecting children from camp.

Please outline any needs your child may have to fully enjoy their camp experience and include any information (such as whether they have an IEP) that may help us to best support them.

Please indicate yes only if you are registering children in the same household.

Important Note: You will need to complete a separate registration for each child in the household.
